This general permission slip is required if your child(ren) wishes to sign up for any JRMS Club or Extracurricular activity during our 2025-2026 school year.

This permission slip only needs to be filled out once per year, per student. You may fill it out now even if your child will not be signing up for any clubs until later this year. 

This permission slip does not sign up your child for any clubs. You must sign up for the clubs following the process linked below after this permission slip is submitted.
